Taking booster vaccination (4th vaccination) of Individual Vaccination (At medical instututions)
Here is information of booster vaccination against COVID-19 (4th vaccination) which is conducted at medical institutions (Individual Vaccination).
※Principally, person registered on basic resident resister of Toyokawa city is eligible.
※Those who can make a reservation or take vaccination are persons whose post 3rd vaccination period is 5 months or more at the point of vaccination day, meeting any of the following 1 - 4.
1. Person aged 60 and older
2. Person aged 18-59 with underlying disease etc.
3. Medical personnel etc.
4. Personnel of senior citizen’s facilities etc.
※For details of "person with underlying disease etc.", see here.
※For person meeting 2, application on website or by phone is necessary for mailing vaccination tickets.

Vaccination starting period and reservation method
※Person who has not received coupon (Vaccination tickets) for booster vaccination (4th vaccination) cannot make a reservation.
For persons aged 60 and older, coupons are sequetially mailed 5 months after the person's 3rd vaccination. Please wait until coupon's delivery.
※For persons aged 18-59 with underlying disease etc., application for vaccination tickets on website or by phone is necessary. See here for details.

Vaccination of mixed vaccine is possible for booster vaccination (4th vaccination)
For booster vaccination, either Pfeizer or Takeda / Moderna vaccine can also be taken regardless of the previous vaccine type.

When you check reservation availability, move on to the following Procedure 2.

Procedure 3. Make a reservation either on website or by phone
Things to prepare when you make a reservation
For confirmation at the point of reservation, prepare delivered coupon (Vaccination ticket etc.) at hand. Prepare also a memo pad to write down reserved date and hour etc.
1. Reservation by phone
Confirm telephone number of desired medical institution on afore-mentioned list of medical institutions and make a reservation.
2. Reservation on website (Available medical institutions only)
Some medical instiltutions accept reservations on website. In case of medical institutions with own reservation website, they can be interlinked with Corona Vaccine Navigation.
Interliked with reservation website for Individual Vaccination.
From the reservation website for Individual Vaccination, log in with your vaccination ticket number (10 digit) and date of birth (8 digit in western year) in half size number. Then, enter your information, change pass word, select desired date and hour, and make a reservation.
Procedure 4. Go to medical institution on the reserved day and hour with necessary documents
Make sure to take the following documents to medical institution on the vaccination day.
- Coupon (Vaccination ticket etc.)
※Keep coupon and medical questionnaire sheet without separation. Please fill medical questionnaire sheet at home and take it to the venue.
- ID (Driver's license, residence card, health insurance certificate etc.)
Medical questionnaire sheet in foreign languages and explanatory document of vaccine (MHLW)
※Fill medical questionnaire sheet in Japanese by referring to the questionnaire sheet in foreign languages.
Precautions after vaccination
- After taking vaccination, you are requested to stay at the vaccination venue for more than 15 minutes (For person with severe allergic history including anaphylactic shock, feeling bad or becoming unconscious in the past, should stay over 30 minutes.). If you notice any negative physical condition, tell the doctor immediately (Medical staff can cope with sudden side reactions.).
- Please keep the injected part clean. Taking a bath on the day is all right but do not rub the injected part.
- Refrain from doing hard exercise for rest of the day.
- Make sure to keep certificate of vaccination.
