Taking booster vaccination (4th vaccination) of Group Vaccination (At Toyokawa Shimin Plaza) 「集団接種（豊川市民プラザ）で追加接種（4回目）を受けるには」

Here is information of booster vaccination against COVID-19 (4th vaccination) which is conducted at Toyokawa Shimin Plaza (Prio 2 4F).

※Principally, person registered on Basic Resident Register of Toyokawa city is eligible.
※Those who can make a reservation or take vaccination are persons whose post 3rd vaccination period is 5 months or more at the point of vaccination day, meeting any of the following 1 - 4.

1. Person aged 60 and older
2. Person aged 18-59 with underlying disease etc.
3. Medical personnel etc.
4. Personnel of senior citizen’s facilities etc.

※For person meeting 2, appllication on website or by phone is necessary for mailing vaccination tickets. See here for details.

Vaccination starting period and reservation method

※Person who has not received coupon (Vaccination tickets) for booster vaccination (4th vaccination) cannot make a reservation.
For persons aged 60 and older, coupons are sequentially mailed 5 months after the person's 3rd vaccination. Please wait until coupon's delivery.

※For persons aged 18-59 with underlying disease etc., application for vaccination tickets on website or by phone is necessary. See here for details.

Schedule

Vaccination of mixed vaccine is possible for booster vaccination (4th vaccination)

For booster vaccination, either Pfeizer or Takeda / Moderna vaccine can also be taken regardless of the previous vaccine type.

Procedure 1. Prepare coupon (Vaccination tickets etc.) at hand

For confirmation at the point of reservation, prepare delivered coupon (Vaccination tickets etc.) at hand. Preapre also a memo pad to write down reserved date and hour etc.

Procedure 2. Make a reservation either on "website" or by "phone"

Reservation for Group Vaccination at Toyokawa Shimin Plaza is available by 2 methods. On the website or Call Center by phone. If there are available date and hour due to cancellation, reseravations are acceptable at any time.
※Reservation acceptance starts from July 1 (Fri.), 2022 at 9:00 am.
※To prevent occurrence of close contact caused by crowd, applications are not accepted at Health Center, and the service is not available if you come. Your understanding is asked.

1. Reservation on website

From the reservation website for Group Vaccination, log in with your vaccination ticket number (10 digit) and date of birth (8 digit in western year) in half size number. Then, enter your information, change pass word, select desired date and hour, and make a reservation.
※In case of cancellation, notifying on the reservation website for Group Vaccination is available. However, if you notify your cancellation 2 days or shorter prior to the reserved day, make sure to contact Call Center (0533-56-2210).

2. Reservation by phone

Note: Reservation cannot be accepted if you directly call Toyokawa Shimin Plaza. You can make a reservation by calling Toyokawa City Novel Corona Vaccine Call Center (0533-56-2210).

Procedure 3. Go to Toyokawa Shimin Plaza on the reserved day and hour with necessary documents

Make sure to take the following documents to Toyokawa Shimin Plaza (Prio 2 4F) on the vaccination day.

• Coupon (Vaccination tickets etc.)

※Keep coupon and medical questionnaire sheet without separation. Please fill medical questionnaire sheet at home and take it to the venue.

• ID (Driver's license, residence card, health insurance certificate etc.)

Make sure to be on time for reserved reception time.

Fill medical questionnaire sheet in Japanese by referring to the questionnaire sheet in foreign languages.

Precautions after vaccination

• After taking vaccination, you are requested to stay at the vaccination venue for more than 15 minutes (For person with severe allergic history including anaphylactic shock, feeling bad or becoming unconscious in the past, should stay over 30 minutes.). If you notice any negative physical condition, tell the doctor immediately (Medical staff can cope with sudden side reactions.).
• Please keep the injected part clean. Taking a bath on the day is all right but do not rub the injected part.
• Refrain from doing hard exercise for rest of the day.
• Make sure to keep certificate of vaccination.
