About 4th vaccination against COVID-19 「新型コロナワクチンの4回目接種を進めています」

※Vaccination against COVID-19 is planned to be conducted until September 30, 2022 (May 25, 2022 currently)

Judging from approach by other countires over lasting period of 3rd vaccination’s effectiveness and effectiveness and safety of 4th vaccination, it was determined that 4th vaccination is conducted from May 25 for the puropose of preventing getting seriously ill in case of getting infected by COVID-19.

Eligibility

Person registered on Basic Resident Register of Toyokawa city whose post 3rd vaccination period is 5 months or more, meeting any of the following 1 -4

1. Aged 60 and older
2. Aged 18-59 with underlying disease etc.
3. Medical personnel etc.
4. Personnel of senior citizen’s facilities etc.

※For person meeting 2., application on website or by phone is necessary for mailing vaccination tickets.
(For details of 2. "person with underlying disease etc.", see the followings.)
※For person meeting 3., application on website is necessary for mailing vaccination tickets.
However, among persons who took 1st vaccination as medical personnel (＝Person who took vaccination with questionnaire sheet attached with vaccination tickets), for person whose post 3rd vaccination period is 5 months or more until the end of July, 2022, application for mailing vaccination tickets is not necessary.
Vaccination tickets for these persons were mailed on July 29 (Fri.).
※For person meeting 4., application on website is necessary for mailing vaccination tickets.
　

For person aged 18-59 meeting any of the following 1-3, 4th vaccination is available.

1. Person whose condition meets any of the following underlying diseases, being outpatient or hospitalized

• Chronic respiratory disease
• Chronic heart disease (Including high blood pressure)
• Chronic kidney disease
• Chronic liver disease (Cirrhosis etc.)
• Under treatment for diabetes with insulin and medicine or diabetes with complications
• Disease of blood (Except iron deficiency anemia)
• Disease causing immunity decline (Including malignant tumor under treatment or palliative care)
• Under treatment with steroids etc. which causes immunity decline
• Neural disease or neuromuscular disease with immune abnormality
• Has physical dysfunction (Such as breathing difficulty) caused by neural disease or neuromuscular disease
• Chromosome abnormality
• Severe physical and mental disability (Combined with severe physical and intellectual disability)
• Sleep Apnea Syndrome (SAS) (Sleeping with breathlessness)
• Severe mental disease (Being hospitalized for treatment, with Mental Disability Certificate (Seishin Shogaisha Hoken Fukushi Techo)(※1), or categorized as "Severe and continuing" under independence support medication (outpatient medication for mental disease), or intellectual disability (With Rehabilitation Handbook (Ryoiku Techo) ※1)

※1 Person with certificate or handbook (Techo) is regarded as person with underlying disease even if the person is not outpatient or hospitalized.

2. BMI is 30 or higher.

3. Possibility of getting seriously ill is high in case of getting infected by COVID-19 from doctor's point of view.

Vaccination Tickets

Vaccination tickets are sequentially mailed 5 months after 3rd vaccination to persons registered on Basic Resident Register of Toyokawa city aged 60 and older who finished taking 3rd vaccination.

※For person aged 18-59 with underlying disease etc., application on website or by phone is necessary for mailing vaccination tickets.

※Vaccination tickets are unified in A3 - sized paper. Although cutline is indicated in the center, take it without separation to vaccination venue.

※Medical questionnaire sheet on the right side needs to be filled out in advance.

※Person who had taken 3rd vaccination in another municipality and moved in Toyokawa city needs to take necessary procedures for mailing vaccination tickets from Toyokawa city. Those who wish to take vaccination are asked to contact Toykawa City Novel Corona Vaccine Call Center (Tel: 0533-56-2210).

Vaccination of mixed vaccine is possible for booster vaccination

For booster vaccination, either Pfeizer or Takeda / Moderna vaccine can also be taken regardless of the previous vaccine type.

Reservation method and vaccination venue

Person who has not received coupon (Vaccination tickets) for booster vaccination (4th vaccination) cannot make a reservation.
For persons aged 60 and older, coupons are sequentially mailed 5 months after the person's 3rd vaccination. Please wait until coupon's delivery.
For persons aged 18-59 with underlying disease etc., confirm here.

1. Person who wishes to make a reservation for medical institution (Individual Vaccination)

2. Person who wishes to make a reservation for Toyokawa Shimin Plaza (Prio 2 4F) (Group Vaccination)
