Group Vaccination is conducted for 1st ・ 2nd vaccination 「【1・2回目】新型コロナワクチンを豊川市民プラザ（集団接種）で接種するには」

Group Vaccination is conducted for persons who have newly become 12 years old and persons who have not taken 1st and 2nd vaccination yet. Those who wish to take vaccination need to make a reservation with coupon at hand.

※Person who has Basic Resident Resister of Toyokawa is principally eligible for the vaccination.

Procedure 2. Make a reservation by "Website" or "Phone".

There are 2 methods to make a reservation for Toyokawa Shimin Plaza (Group Vaccination). Website or Call Center.
If there is available hour due to cancellation, reservations can be accepted at any time.

※Right after reservation starting date, phone lines and website get very busy with calls and access. You may not be able to make a reservation and be asked to wait for a while, but vaccination is available for all persons who wish to take. Please have some extra room for your mind and time to make a reservation.

1. By Website

Log in by using the vaccination ticket number (10 digits) and number of your birth day with western year (8 digits) (Half size number). Then, type your information, change the pass word, select desired date and time, and make a reservation.
※In case of canceling, it can be done on the reservation website for Group Vaccination, but if the cancellation is within 2 days prior to the vaccination day, make sure to contact Call Center (0533-56-2210).

2. By Phone

※Aside from Japanese, corresponding languages are English (Monday - Friday), Chinese (Thursday), and Portuguese (Friday).
Note: Making a reservation by directly calling Toyokawa Shimin Plaza is not available. You can make a reservation with this number of Call Center (0533-56-2210).

Procedure 3. Go to Shimin Plaza with necessary documents on the reserved date and time.

Please make sure to take the following documents to Toyokawa Shimin Plaza (Prio 2 4F) on the vaccination day.

• Coupon (vaccination tickets etc.) ※Please keep it without separation.
• Medical Questionnaire ※Fill it out at home and take it with you. Person aged 15 years and younger needs guardian's signature.
• IDs (Such as driver's license, residence card and National Health Insurance Certification etc.)

Make sure to be on time for reserved reception hour.
※Guardian must accompany the person aged 15 years and younger.

By using medical questionnaire form in foreign languages on the website of Ministry of Health, Labor and Welfare as an example, fill out your medical questionnaire form written in Japanese.

Precautions after vaccination

• After taking vaccination, you are requested to stay at the vaccination venue for more than 15 minutes (Persons with severe allergic history including anaphylactic shock, feeling bad or becoming unconscious in the past, should stay over 30 minutes.). If you notice any negative physical condition, tell the doctor immediately (Medical staff can cope with sudden side reactions.).
• Please keep the injected part clean. Taking a bath on the day is all right but do not rub the injected part.
• Refrain from doing hard exercise for rest of the day.
• Please keep vaccination certificate for sure.
