How to take vaccination against COVID-19 at medical institutions (Individual Vaccination) 「新型コロナワクチンを医療機関（個別接種）で接種するには」

Start of reservation acceptance of Individual Vaccination for persons with underlying disease, persons working in senior citizen's facilitites etc. and persons who have Physically Disabled Person's Handbook (Vaccination at medical institutions): Starting sequentially at respective medical institutions

Request to those who make reservations ※Please make sure of the right number when you make a phone call.
※Starting date of reservation for persons with underlying disease, persons working in senior citizen's facilities etc. and persons who have Physically Disabled Person's Handbook is different at respective medical institutions depending on the process of vaccination for senior citizens. Please check it with Corona Vaccine Navigation.
※Right after start of reservation acceptance, telephone lines and website for reservation get very busy. You may not be able to make a reservation and be asked to wait for a while, but vaccination is guaranteed for every person who desires it. Please have some room in your time and mind, and make a reservation without haste .
※Depending on the medical institutions which take priority for the regular patients concerning vaccination, there may be occasions that only the reservations from regular patients can be accepted right after the start of reservation.
※To avoid the occurrence of 3 Cs (Closed space, Crowd, and Close contact), reservation acceptance at Health Center's booth is not conducted. Please understand that corresponding to your request is not possible even if you come to the Center.

In case of taking vaccination against COVID-19 at medical institutions, reservation in advance is necessary with the following procedure.
※For persons with underlying disease, check here.

Procedure 1. Check the reservation state of elibible medical institutions from "Corona Vaccine Navigation"(In Japanese only).

• Available reservation time frame at registered medical institutions can be checked with this "Corona Vaccine Navigation".
• You can not directly make a reservation with "Corona Vaccine Navigation. When you can check reservation availability, move on to Procedure 2

Procedure 2. Choose an available medical institution (Which you are eligible for) and make a reservation by phone.

Things to prepare

For confirming relevant information on the phone, prepare the delivered coupon (vaccination tickets etc.) at hand.
Please prepare a memo pad to make sure of your reservation date and time.

Procedure 3. Go to the medical institution with necessary documents on the reserved date and time.

Please make sure to take the following documents to the medical institution on the vaccination day.

• Coupon (vaccination tickets etc.) ※Please keep it without separation.
• Medical Questionnaire ※Fill it out at home and take it to the medical institution.
• IDs (Such as driver's license, residence card and National Health Insurance Certification)

By using medical questionnaire form in foreign languages on the website of Ministry of Health, Labor and Welfare as an example, fill out your medical questionnaire form written in Japanese

Procedure 4. (In case of taking 1st vaccine produced by Pfizer) Make a reservation for the 2nd vaccination.

• Vaccine of Pfizer needs to be taken twice
• After taking 1st vaccination, take the second one with 3-week interval.
• If you take Pizer's vaccine on the 1st vaccination, make sure to take the same Pfizer's vaccine on the 2nd vaccination.

Precautions after vaccination

• After taking vaccination, you are requested to stay at the vaccination venue for more than 15 minutes (Persons with severe allergic history including anaphylactic shock, feeling bad or becoming uncnsious in the past, should stay over 30 minutes.). If you notice any negative physical condition, tell the doctor immediately (Medical staff can cope with sudden side reactions.).
• Please keep the injected part clean. Taking a bath on the day is all right but do not rub the injected part.
• Refrain from doing hard exercise for rest of the day.
• Please keep vaccination certificate for sure.
