Eligibility
Principally, person registered on basic resident resister of Toyokawa city.
※For persons living in other municipalities, vaccination is not available except some specific cases. Contact Call Center for details.
※Those who can make a reservation and take 3rd vaccination are limited (Person aged 12 years and older whose post 2nd vaccination period is principally over 6 months.).

Vaccination starting period and reservation method
Coupons of booster vaccination (3rd vaccination) are sequentially sent 6 months after 2nd vaccination. Please make a reservation after coupon is delivered.
※Person who has not received coupon cannot make a reservation.

Vaccination of mixed vaccine is possible for booster vaccination (3rd vaccination)
Person who has taken vaccine of Pfizer for 1st and 2nd vaccination can also take vaccine of Moderna now for booster vaccination (3rd vaccination).
Procedure 1. Check reservation availability of medical institutions on "Corona Vaccine Navigation"

Procedure 2. Choose available medical institution (matching your eligibility)
Procedure 3. Make a reservation either on website or by phone
For confirmation at the point of reservation, prepare delivered coupon (Vaccination ticket etc.) at hand. Prepare also a memo pad to write down reserved date and hour etc.
1. Reservation by phone
Confirm telephone number of desired medical institution on afore-mentioned list of medical institutions and make a reservation.
2. Reservation on website (Available medical institutions only)
Some medical instiltutions accept reservations on website. In case of medical institutions with own reservation website, they can be interlinked with Corona Vaccine Navigation.
Interliked with reservation website for Individual Vaccination.
From the reservation website for Individual Vaccination, log in with your vaccination ticket number (10 digit) and date of birth (8 digit in western year) in half size number. Then, enter your information, change pass word, select desired date and hour, and make a reservation.
Procedure 4. Go to medical institution on the reserved day and hour with necessary documents
Make sure to take the following documents to medical institution on the vaccination day.
- Coupon (Vaccination ticket etc.)
※Keep coupon and medical questionnaire sheet without separation. Please fill medical questionnaire sheet at home and take it to the venue.
- ID (Driver's license, residence card, health insurance certificate etc.)
Make sure to be on time for reserved reception time.

Precautions after vaccination
- After taking vaccination, you are requested to stay at the vaccination venue for more than 15 minutes (For person with severe allergic history including anaphylactic shock, feeling bad or becoming unconscious in the past, should stay over 30 minutes.). If you notice any negative physical condition, tell the doctor immediately (Medical staff can cope with sudden side reactions.).
- Please keep the injected part clean. Taking a bath on the day is all right but do not rub the injected part.
- Refrain from doing hard exercise for rest of the day.
- Make sure to keep certificate of vaccination.
