Vaccination Certificate of COVID-19 「新型コロナワクチン接種証明書について」

Vaccination Certificate of Covid-19 can be issued from end of July for overseas use. Since vaccination certificate or vaccination record slip which can be obtained currently is not apporpriate for overseas use without English translation etc., the new certificate is the one issued for this purpose.
※The document issuance may take several days depending on how the person's vaccination record is reflected on the vaccination record system.

What is Vaccination Certificate of COVID-19 (Vaccination Passport)?

Currently, Certificate of Vaccination for COVID-19 (Which is on the right side of coupon) is issued on vaccination against COVID-19. The certificate does not have English translation and preventive measures against counterfeit, so it is not appropriate for overseas use. Therefore, as a document which proves the fact of having taken vaccination against COVID-19 ("Vaccination Passport" hereafter) is issued. It is expected that infectious preventive measures will be eased as shortened quarantine segregation period and exemption of submitting PCR Test Negative Certificate before departure by showing the Vaccination Passport. About entry restrictions and conditions for entry / movement restrictions in other counntries at this point, see Overseas Safety HP of Ministry of Foregn Affairs. Although infectious preventive measures can be partially eased by showing Vaccination Passport, other restrictions may continue to be applied.

Method of Vaccination Passport issuance

Necessary documents for application

Following documentts are necessary for application.

• Application form (Can be downloaded from line-up of respective forms.)
• Passport (Note 1)
• Vaccination ticket (See the image) (Note 2)
• Certificate of Vaccination for COVID-19 or Record of Vaccination for COVID-19 (Which is issued when the person takes vaccination at work etc. without coupon from municipalities.) (Note 3)

In case of applying at responsible booth, please show necessary documents. If you apply by postal mail, enclose copies of necessary documents. Please understand that submitted documents are principally not returned (If returning documents is requested, please mention it and enclose a return envelope and stamp, or put stamp on the return envelope.).
Note 1: Please check the validity period of your passport (In case of expired period, the certificate issuance is not available.). Please show the passport which is to be accompanied by Vaccination Passport for overseas use.
Note 2: In case of having lost vaccination ticket or having used " medical checkup-only" ticket, bring My Number confirmation documents (My Number Card, Notification Card, or a copy of resident certification with individual number).
Note 3: Even in case of having lost Certificate of Vaccination for COVID-19 or Record of Vaccination for COVID-19, the issuance of Vaccination Passport is possbile. However, it may take time to confirm your vaccination record.

Person meeting the conditions written below needs the following documents.
If former family name / another family name / another name is written on the passport,

• ID documents which can indicate former family name / another family name / another name (My Number Card with former family name / another family name / another name, driver's license, passport of another country etc.)

In case of application by agent

• Proxy with the applicant's signature (The form can be downloaded from the line-up of respective forms.)

Application by postal mail

• Return envelope on which delivery address is written and stamp is put
• Copy of ID with address

Where to apply

It is necessary to apply in the municipality which issued the person's vaccination ticket. In case that 1st and 2nd vaccination tickets were issued by two municipalities, application is necessary in respective municipaliaties.
It is free of charge. One certificate is issued per application.

It may take time till the issuance of Vaccination Passport. Especially if it is applied right after taking vaccination, it may take time to confirm the person's vaccination record. Please come with enough room for your time.

Q1: In which countries can be used Vaccination Passport?

A1: Currently, information gathering is going on concerning certificate of vaccination and restriction easing measures of other countries for acceptance of Vaccination Passport issued by the government of Japan and easing infectious preventive measures. Eligible countries, regions, and their restriction easing measures are updated on HP of Ministry of Foreign Affairs in a timely manner.

Italy, Austria, Turkey, Bulgaria and Poland (July 21, 2021 currently)
※In case of South Korea, Vaccination Passport is recognized as "Certificate of Vaccination" which is one of the necessary documents for issuing exemption of quarantine segregation document.
※For the latest and accurate information of exemption and easing measures which are actually applied, see HP of Ministry of Foreign Affairs.

Q2: Can Vaccination Passport be used domestically for trips and using restaurants?

A2: For the time being, Vaccination Passport is expected to be used as a document to authorities of other countries for easing infectiious preventive measures at the time of entry. Domestically, the fact of having taken vaccination can be confirmed with vaccination certificates (Which are originally attached to the right side of vaccination tickets).

Q3: I lost vaccination certificates. Can I apply for Vaccination Passport to substitute it for the lost vaccination certificates?

A3: Vaccination Passport and vaccination certificates are the documents of different legal classification. Without Vaccination Passport, it is domestically possible to confirm the fact of having taken vaccination with vaccination certificates. Vaccination certificates can be reissued. Q4: I have not taken vaccination against COVID-19 (Cannot take it). Without Vaccination Passport, don't I get discrimination?

A4: It is considered that Certificate of Vaccination is just to prove the fact of the person having taken vaccination and the certificate's issuance itself would not cause discrimination. About prevention of discrimination or having received discriminative treatment, please see here.
The difference of infectious preventive measures of respective countries is now under investigation. About eligible countries and regions in which Vaccination Passport can be used, and their restriction easing measures, relevant information is updated on Overseas Safety HP of Ministry of Foreign Affairs in a timely manner.
