How to take vaccination against COVID-19 at Toyokawa Shimin Plaza (Group Vaccination) 「新型コロナワクチンを豊川市民プラザ（集団接種）で接種するには」

Request to those who make reservations ※Please make sure of the right number when you make a phone call.
※Starting date of reservation for persons with underlying disease, persons working in senior citizen's facilities etc. and persons who have Physically Disabled Person's Handbook is different at respective medical institutions depending on the process of vaccination for senior citizens. Please check it with Corona Vaccine Navigation.
※Right after start of reservation acceptance, telephone lines and website for reservation get very busy. You may not be able to make a reservation and be asked to wait for a while, but vaccination is guaranteed for every person who desires it. Please have some room in your time and mind, and make a reservation without haste.
※Depending on the medical institutions which take priority for the regular patients concerning vaccination, there may be occasions that only the reservations from regular patients can be accepted right after the start of reservation.
※To avoid the occurrence of 3 Cs (Closed space, Crowd, and Close contact), reservation acceptance at Health Center's booth is not conducted. Please understand that corresponding to your request is not possible even if you come to the Center.

About Group vaccination at Toyokawa Shimin Plaza (Prio 2 Bldg. 4F), except holidays, vaccination is conducted on the day with 〇.
Vaccination is conducted in the time frame of 30 minutes. Please make a reservation for your desired hour.

Schedule of Group Vaccination

Procedre 2. Make a reservation by "Website" or "Phone".

There are 2 methods to make a reservation for Toyokawa Shimin Plaza (Group Vaccination). Website or Call Center.

1. By Website

※Website reservation is currently available only for persons aged over 65 years. Persons aged under 64 years are asked to wait until the starting date of reservation. Starting date of reservation for persons aged under 64 years is updated on Toyokawa City HP or Toyokawa Newsletter August Edition.

If you click the 3 white lines on the top right of reservation site in Japanese (The image on the left), you will see the reservation site in other languages (The image on the right).

Log in by using the vaccination ticket number and 8 figure of your birth day with Christian Era (Half size number). Then, change the pass word, select desired date and time, and make a reservation.

2. By Phone

Note: Making a reservation by directly calling Toyokawa Shimin Plaza is not available.

※Only for persons with impairment of hearing and speaking Reservation by Fax

Persons who have difficulty in applying by phone due to impairment of hearing and speaking etc. can apply for vaccination by FAX. Please fill the form shown below to fax.
※This is the application method only for persons with impairment of hearing and speaking. If an application by FAX is found to have been sent by inelibible person, reservation will be canceled.

Procedure 3. Go to Shimin Plaza with necessary documents on the reserved date and time.

Please make sure to take the following documents to Toyokawa Shimin Plaza ( Prio 2 Bldg. 4F) on the vaccination day.

• Coupon (vaccination tickets etc.) ※Please keep it without separation
• Medical Questionnaire ※Fill it out at home and take it with you.
• IDs (Such as driver's license, residence card and National Health Insurance Certification)

Make sure to be on time for reserved reception hour.
(Ex. If your reservation is for the time frame 10:30 am - 11:00 am, please come by 10:30.)

By using medical questionnaire form in foreign languages on the website of Ministry of Health, Labor and Welfare as an example, fill out your medical questionnaire form written in Japanese.

Procedure 4. (In case of taking 1st vaccine produced by Pfizer) Make a reservation for the 2nd vaccination.

• Vaccine of Pfizer needs to be taken twice.
• After taking 1st vaccination, it is necessary to take the second one with 3-week interval. The date and time for the 2nd vaccination is informed at the venue (If the informed date is not convenient for you, arrangement is tried for another date.).
• If you take Pfizer's vaccine on the 1st vaccination, make sure to take the same Pfizer's vaccine on the 2nd vaccination.

Precautions after vaccination

• After taking vaccination, you are requested to stay at the vaccination venue for more than 15 minutes (Persons with severe allergic history including anaphylactic shock, feeling bad or becoming unconscious in the past, should stay over 30 minutes.). If you notice any negative physical condition, tell the doctor immediately (Medical staff can cope with sudden side reactions.).
• Please keep the injected part clean. Taking a bath on the day is all right but do not rub the injected part.
• Refrain from doing hard exercise for rest of the day.
• Please keep vaccination certificate for sure.
