Person who takes vaccination against COVID-19 in another municipality (Not in Toyokawa) 「新型コロナワクチンを住所地外（市外）で接種される方へ」

Principally the vaccination against COVID-19 is to be taken at medical institutions or vaccination venues of Group Vaccination in the municipality of the person's residence certification (Jyumin Hyo).
However, person with the following reasons 1-9 can take the vaccination in another municipality.
※In case that the person belonging to another municipality takes vaccination in Toyokawa, medical institutions (Individual Vaccination) are the only eligible places. Group Vaccination is not available.

In case that notification in advance is not necessary

Person with the following reasons 1- 4 can take vaccination in another municipality by reporting to the doctor who implements the person's vaccination. In this case, notification to the municipality is not necessary.
1. Person who takes vaccination at medical institutions or facilities of another municipality where the person is currently staying.
2. Person who takes vaccination at the medical institution where the person is taking treatment for its underlying disease
3. Victim of disasters (Those who live in other municipalities due to nuclear power plant disaster caused by Great East Japan Earthquake can take vaccination by using vaccination tickets sent from the municipality of the person's evacuation destination.)
4. Person who is in custody or in jail

In case that notification in advance is necessary

Person with the following reasons 5-9 can take vaccination in another municipality which is not the person's regisgtered address by notifying the municipality in advance where the medical institution of the person's vaccination is located.
5. Business bachelor
6. Student living away from home
7. Victim of DV, stalking etc., or child abuse and relevant behaviors
8. Pregnant woman staying in her hometown for childbirth
9. Person whose situation is confirmed by the mayor of the municipality as inevitable to take vaccination in another municipality

To persons who needs to do notification of vaccination in another municipality

In case that person who is a resident of Toyokawa wishes to take vaccination in another municipality with afore-mentioned reasons 5-9, or person whose registered address is not in Toyokawa wishes to take vaccination in Toyokawa, "Notification of Vaccination in Another Municipality (Jyushochi Gai Sesshu Todoke)" is necessary.
Notification procedures are as follows.
(1) Open the website of "Corona Vaccine Navigation" and select the desired municipality on the "Notification of Vaccination in Another Municipality".
(2) Enter the vaccination number of the delivered coupon, then, select the municipality of residence certification (Jyumin Hyo), vaccination process and the reason of notification.
※If you select "Living in the municipality due to inevitable reasons", enter the reason as well.
(3) After confirming terms of service and privacy policy, mark ✓for "Agree" and select "Confirming the content of entry".
(4) After confirming the content of entry and no errors to be found, select "Apply".
(5) "Certificate of Notification of Vaccination in Another Municipality (Jyushochi Gai Sesshu Todokede Shou)" is shown. Make sure to print it out, or take a screen shot (In case of smartphone) and keep it.
※"Certificate of Notification of Vaccination in Another Municipality" which is shown after completing the notification on Corona Vaccine Navigation is displayed only once. If you close the site before printing out or screenshot (By smartphone), re-displaying is not available. In this case, notification procedures must be taken from the beginning.
(6) On the reserved vaccination day, "Certificate of Notification in Another Municipality" needs to be shown at the reception of medical institution etc. It is necessary for both the 1st vaccination and the 2nd vaccination.

Note

• Please understand that mail delivery or informing about this matter from relevant organization will not be done after you finish "Notification of Vaccination in Another Municipality" on Corona Vaccine Navigation.
• Principally, the printed out "Certificate of Notification of Vaccination in Another Municipality" is not collected at medical institutions. The certificate which you show on the 1st vaccination should be kept for the 2nd vaccination.
