For those who missed the opportunity to take 2nd vaccination of COVID-19 by Moderna due to poor health condition etc., Aichi prefecture opens 4 venues where the vaccine of Moderna can be taken (Moderna Vaccination Center).

Vaccination Scale
Once a week at respective medical institutions (Around 20 persons)
Eligible Person
(1) Person who took 1st vaccination by Moderna at large-scale vaccination venue in Aichi and has not taken 2nd vaccination yet.
(2) Person who took 1st vaccination by Moderna at workplace vaccination in Aichi and has not taken 2nd vaccination yet.
(3) Person who took 1st vaccination by Moderna abroad or in another prefecture and has not taken 2nd vaccination yet (Limited to persons who live, study or work in Aichi.).
Starting Date of Reservation (In common with respective medical institutions)
November 26 (Fri.), 2021 from 9:00 am (From this date, reservations for respective week will be accepted at 9:00 am every Friday.)

※Vaccination tickets sent from the municipality of your residence are required for reservation.
※Medical institutions are not accepting reservations. Please refrain from contacting medical institutions.
Things to bring on the day
(1) Vaccination Tickets (To confirm 1st vaccination record, Certificate of Vaccination for COVID-19 or Vaccination Record Slip is necessary.)
(2) ID (My Number Card, driver’s license, residence card or health insurance certificate etc.)
(3) Health Insurance Certificate (In case of necessary medical treatment for poor health condition after vaccination)
Others
Vaccine for COVID-19 and other type of vaccine cannot be taken at the same time. Make sure to take vaccinations with intervals of more than 2 weeks.
