2. Person aged between 60 and 64 with cardiac, renal, respiratory disorder etc. who has Physically Disabled Person’s Handbook Level 1 (Shintai Shogaisha Techo)

(Person who took vaccination against pneumococcus (23 valent) in the past, is not eligible.)

1. The following month of 65-year-old birth month
2. June

2,000 yen (Free of charge for resident tax exempted household and household on welfare)

Medical questionnaire sheet, things which indicate address etc. (My Number Card etc.)

Make a reservation directly to registered medical institutions written on the backside of the information leaflet.

For person who became 65 in the fiscal year 2023 and has not taken vaccination yet, the eligibility is until previous day of the 66-year-old birthday. The questionnaire sheet mailed in the fiscal year 2023 is not usable. Please apply for reissuance on city website or by phone (Tel: 0533-95-4801).
