What is periodic vaccination?
Periodic vaccination is implemented by residence municipalities, and respective vaccine has eligibility, frequency, and intervals between vaccinations. Vaccination at commissioned medical institutions in Toyokawa is free of charge. In case of health damage caused by vaccination, there is compensation for it by Ministry of Health, Labour and Welfare. If the applicant does not have general practitioner (Family doctor) in Toyokawa and wishes to take vaccinations at medical institutions in other municipalities, it is necessaray to take procedures in advance at Toyokawa Health Center (Toyokawa Hoken Center). (Without taking procedures at Toyokawa Health Center in advance, vaccinations in other municipalities are not regarded as Periodic Vacciantion and charge will occur.)

*2 weeks before or after taking vaccination for COVID-19, other vaccinations cannot be taken.

Things to bring
- Medical questionnaire sheet for the vaccination (It is mailed to the address of the person's residence certification. It can be reissued at Health Center if it is lost.)
- Maternity passbook (Boshi Techo) (Without maternity passbook, vaccination is not available. If it is lost, conatct Health Center.)
- ID which indicates the person's address (Child Medical Expenses Recipient Certificate "Kodomo Iryohi Jukyusha Sho", My Number Card etc.)
*Proxy (free format) is necessary if 3rd person other than guardian accompanies the child.
