How to file notification of death (Shibo Todoke)

When a person dies in Japan, the person’s death must be reported within 7 days after the day the death was known. The reporting person goes to city hall of deceased person’s residence with death certificate (Shibo Shindan Sho) issued by a doctor to file the notification.

After that, if there are residence card (Zairyu Card) and National Health Insurance Certification (Kokumin Kenko Hoken Shou) etc., they need to be returned. In addition, please file the notification likewise at the consulate (embassy) of the deceased person’s country as well.
