Information concerning damage caused by heavy rain in June 2023 (About reduction etc. of National Health Insurance Copayment) 「令和5年6月 大雨による被害に関するお知らせ(国民健康保険一部負担金の減免等について)」

In case that housing or land (which person insured by National Health Insurance lives in or owns) received damage caused by heavy rain from June 2 (Fri.) to 3 (Sat.), 2023, and the person is in financial difficulty not being able to pay the copayment, there is copayment reduction system which reduces the person’s self-borne expense at medical institution etc.
About reduction etc. of National Health Insurance Copayment
Eligible person
1.Person whose income is certain level or lower died, became physically/mentally disabled, or received serious damage to its property due to earthquake, storm, flood, fire, or other disasters of similar kind
※Inundation under floor level is not eligible.
2. Person whose income is certain level or lower had noticeable decrease in its income due to suspension/closure of business, noticeable loss in its business, or unemployment
3. In case that mayor recognizes it necessary besides afore-mentioned clause 2

1. Application Form of National Health Insurance Copayment Reduction/Exemption/Collection Grace Period (Kokumin Kenko Hoken Ichibu Futankin Gengaku Menjo Choshu Yuyo Shinseisho)
2. Livelihood Report Form of All the Household Members
3. Income Certificate (Kyuyo Shomeisho)
4. Other certificates of public organization etc. necessary for proving the reason for application (Certificate of Disaster Victim (Risai Shomeisho) etc.)
Others
Approval/disapproval of reduction etc. is judged by the damage condition and income situation of the household.
