Identification booklets are required to take advantage of these services. There are three classifications that are physical, intellectual and mental disabilities. If you become disabled or your child is born with a disability, apply for the identification booklets at the Welfare Division. To apply for physical and mental disability booklets, specific medical certificates are required. Please ask the Welfare Division for more information on which certificates you require.
To apply for intellectual disability booklets, have a screening interview at East Mikawa Child and Disability Consultation Center; appointments can be made through the Welfare Division. Various services are available after acquisition of the booklets such as home visits, home bath services, short-stay services, day service, nursing homes, allowances, deduction of tax, exemption of medical expense, etc The services that you are entitled to depend on your degree of disability and personal income.
